Complete your Right of Entry Authorization
The EquiFlow Lead-Free Program is working with the Town of Westerly to remove lead service lines and upgrade aging water infrastructure. To ensure your service line can be replaced when work reaches your area, a completed Right of Entry (ROE) Authorization Form must be on file.
The ROE grants permission for the Town of Westerly, the EquiFlow team, and town-approved contractors to access your property and replace the portion of the service line on your property. Submitting this form allows the public and private sections of the line to be replaced at the same time, helping keep construction coordinated and efficient.
Only the property owner or an authorized representative may sign the ROE. If someone else is signing on the owner’s behalf, such as a tenant, family member, or property manager, they must include a notarized letter or legal documentation confirming their authority.

Use the form below to grant permission for the Town of Westerly, the EquiFlow team, and town-approved contractors to access the property for lead service line replacement activities. Review the information carefully, complete all required fields, and sign electronically.
Once your form is received, the EquiFlow team will review your information and contact you when construction is approaching your neighborhood, typically one week in advance. Having your ROE on file ensures your property is included in planning and avoids delays when work begins.
